Kasan Moorehead

With over 25 years of dedicated experience in nursing and the healthcare industry, Kasan “Kasey” Moorehead is a highly passionate, patient-centered advocate with a wealth of knowledge as a Care Manager and owner of Care Solutions In-Home Services serving the adult, senior, and disabled populations. She is an Advanced Aging Life Care Professional specializing in care coordination, patient advocacy, and home care services.

Kasey’s participation in the Missouri's Nurse Licensure Compact, as well as her license in California and Illinois, allows her reach to span across the United States with the goal of providing care coordination and advocacy to clients in need, wherever they reside. Her essential focus is to provide comprehensive care management while ensuring that clients achieve optimal health outcomes by aligning them with adequate care coordination services and resources to promote their health and wellness goals. Kasey’s sound clinical judgment and critical thinking skills are reflective of her nursing experience in various healthcare settings, enabling her to anticipate the needs of her clients.

Her expertise has played a critical role in reducing hospitalizations and readmissions for her clients through proactive care management, promptly identifying and addressing potential gaps in the continuum of care, aligning clients with supportive care services, and facilitating smooth transitions between care settings.
